I/We do hereby consent and give Creation Festivals, LLC. and their assigns, licensees and legal representatives the irrevocable right to use my name (or any fictional name), picture, portrait, visual image or photograph (the "Materials") in all forms and media, now known and hereafter created, and in all manners, including composite or distorted representations, for advertising, trade or any other lawful purpose, and I waive any right to inspect or approve the finished product, including written copy that may be created in connection therewith and in any derivative work based thereon (the "Work") throughout the world in perpetuity and to use them to publicize, promote and advertise the Work in any manner.
